*porsche*, 23 July, 2012 at 11:47 Posted on Planning 0 10

Is there such a thing, know the obvious, passport, banks, driving licence, just wondered if there was a list somewhere I could check off. ?

Thanks Lanny, I have copied and saved your list for future reference! Just made a head start with it by ordering the passport change of name form, everything else can wait except that!

I don't know if anyone will find this useful for when you need to send a letter to get your name changed:

RE: Notification of Change of Name - YOUR POLICY NUMBER/REFERENCE NO

I am writing to inform you that following my marriage on [DATE]my name has changed from [YOUR PREVIOUS FULL NAME]to [YOUR NEW FULL NAME].
I am pleased to enclose a copy of my marriage certificate as evidence. Should you require to see the original certificate, please let me know.

Please change your records to show my amended personal details. For your records my old and new signatures are shown below.

    Yours sincerely,

Old name: Print or type your old name here, in full and in CAPITAL LETTERS then sign your old name underneath in blue or black ballpoint or fountain pen

New name: Print or type your new name here, in full and in CAPITAL LETTERS then sign your new name underneath in blue or black ballpoint or fountain pen.

With regard to HMRC, surely this down to the bosses to do isnt it, when they update the contract, etc. Most companies will only acept the original marriage certificate wont they and not a photocopy. How does it work. I haven't got a clue and there is a lot to change over.

I am now confused. I am changing my name at the bank etc but keeping my maiden name for professional purposes so not sure how this will work with HMRC and getting paid.

Would all my contracts say married name but business card maiden?
