Cannot pick a location

Hey everyone! So basically me and my girlfriend have been engaged for years (since 2016) but we are struggling to find a location. Both of our families are from Italy, and the places aren’t technically too far (1h with the plane, Naples and Palermo) but I have a tiny family (of which two grandmas who are too ill and weak to travel) while my girlfriend has a huge family (at least 50 people?) but they cannot afford to travel.
I really want my grandmas there but at the same time if we do it in Palermo, where I am from, she will lose on half her family. She isn’t too bothered about most of them but there are a few people she really care about being there. I’d offer to pay for them but we are already struggling to put together wedding money (that’s why it’s taking so long for us to get married). Also even if we did pay for them some of them cannot travel (one is agoraphobic and other people have small kids or cannot get more than a day off work). Having two cerimonies is something we could do but the one in Palermo would be tiny because I have a tiny family so I feel like it would be quite boring? I’m so lost honestly I don’t know what to decide
