need help getting affidavit for south africa 2023

Hi All, My Irish partner(has lived in UK for many years) and I are trying to get married in Cape Town in 2023. My friend over there has been a star and organised venue, flowers and civil wedding officer but we need to get an affidavit in place of a Certificate of Non Impediment (UK does not issue these for Commonwealth countries). The notary publics I have spoken to will not witness signing unless we have the exact wording required by the South African Home Affairs. Anyone know how or where to get the right form please?
