Who do I supply buttonholes/boutonnieres to? Everyone attending the ceremony or just the top few/wedding party

Usually just the wedding party. Although providing for parents can also be a nice gesture.

Yeah, usually just the wedding party. But, can be a nice touch for parents too! If you get a dried flower one they can keep them as a memento of the day.
